Geriatric Medicine Consult Team
In February 2025, a collaborative effort was made to bring together Geriatricians, Geriatrics Nurse Specialists, and Geriatrics Registered Nurses to form a unified Geriatric Medicine Consult Team.
This newly established, interdisciplinary team is dedicated to supporting patients aged 65 and older, addressing a wide range of complex issues including cognitive impairment, delirium, multiple chronic medical conditions, polypharmacy, functional decline, falls, caregiver distress, and decision-making challenges related to frailty and dementia.
In addition to providing direct patient care, the team plays a vital role in enhancing the knowledge and skills of staff at QCH by offering education and capacity-building initiatives for both physicians and other healthcare professionals. The team’s responsibilities include conducting joint case reviews, triaging patients daily, and performing comprehensive geriatric assessments on all individuals referred to their service. They thoroughly document their findings and provide detailed recommendations to ensure the best possible care for our geriatric patients.
